I thought this subject needed it’s own article its so important.
As the GLP1 meds become more common and there’s hand waving and “oh you can take it anytime” type of talk. That type of talk minimizes risk to make you feel better but you need to be aware anyway.
Among patients on GLP1s undergoing endoscopy, one large study found retained gastric contents in patients 13.6% versus 2.3% in previous studies— a roughly four-fold increase. Actual aspiration remained rare.
2024 multi-society guidance says most people should continue their GLP1 before elective surgery — a deliberate softening of stricter 2023 advice. But highest-risk patients (those escalating, those with active GI symptoms, those on higher doses) may be advised a 24-hour clear-liquid diet beforehand. You may also need to stop eating sooner to achieve needed empty stomach. Work with your doctor and talk about it to be sure you’re clear what to do.
It’s recommended you do not stop the drug on your own before a procedure. Tell them about the GLP1 and ask questions. That includes dental work under sedation and colonoscopy.
Trip to Endodontist
I did let my endodontist know I’m on a GLP1 med. We talked about an arthritis med I’m on instead. And that he wants me to skip the medicine. Serious stuff, but I think my issue with nausea could make the GLP1 med more important depending on when I take it and when I’m scheduled for my appointment. I plan to follow up with him before the procedure to make sure I know what to do. I may end up delaying my dose a few days to be sure I don’t have any issues during surgery. But I will make the decision with my doctor and follow it.
